Job Summary
Due to the recent development of Assessment and Liaison Services, Rotherham Doncaster and South Humber NHSFT offers an opportunity for a forward-thinking, highly motivated, innovative, dedicated and experienced Social Worker to join the Crisis Resolution and Home Treatment (CRHT) Team in North Lincolnshire.
The CRHT is a multi-disciplinary team providing 24/7 urgent clinical triage and advice via a crisis telephone line, as well as conducting urgent and emergency mental health assessments for people in crisis. The team also provides intensive home treatment as an alternative to hospital admission.
The CRHT acts as gatekeeper for inpatient admissions, crisis house admissions, and facilitates early discharge from inpatient wards.

The role includes a robust Trust induction, and continuing professional development is supported through monthly Trust Learning Half Days.

Main Duties of the Job
A motivated and skilled Social Worker is required to join the Crisis Resolution and Home Treatment Team (CRHT) in North Lincolnshire.
Working within a 24/7 multi-disciplinary service, the role includes:
- Providing urgent mental health triage, crisis assessment, and intensive home treatment as an alternative to hospital admission
- Acting as gatekeeper for inpatient and crisis house admissions
- Supporting early discharge and ensuring safe, person-centred care
Working closely with health, social care, and community partners, the role involves:
- Applying the Care Act, Mental Health Act, and Mental Capacity Act in practice
- Completing high-quality assessments and risk management plans
- Coordinating care packages and promoting recovery-focused, strengths-based practice
- Supporting individuals, carers, and families
- Contributing to safeguarding and multi-agency working
- Supporting development of social work practice within the service

- ASYE or equivalent experience
- Social Work qualification (DipSW, BA Hons, BSc Hons, MA or equivalent)
